The Patient Health Questionnaire (PHQ) is a series of self-report screening tools designed to assess various mental health conditions, such as depression, anxiety, and somatic symptoms.

  • PHQ-2– A two-item questionnaire that screens for depression.
  • PHQ-4 – A four-item questionnaire that screens for both depression and anxiety.
  • PHQ-9 – A nine-item questionnaire that screens for depression and assesses its severity.
  • PHQ-15 – A fifteen-item questionnaire that screens for somatization and somatic symptom severity.

Generalized Anxiety Disorder (GAD) Questionnaires

  • GAD-2 and GAD-7 are two and seven item questionnaires that screen for generalized anxiety disorder and assesses its severity.

Somatic, Anxiety, and Depressive Symptoms (SADS) Questionnaire

  • PHQ-SADS – A combination of the PHQ-9, GAD-7, and PHQ-15 to assess depression, anxiety, and somatic symptoms.
